WORKERS' COMPENSATION CERTIFICATE

Provide a photo copy of your current certificate.   An updated copy is required every six months.   You may apply for an Ohio Workers' Compensation number at 1-800-644-6292.

INDEMNITY AGREEMENT

Sign and return two (2) copies of the indemnity agreement to our office.  When you have met all of the requirements, a completed copy will be returned to you.

Buckeye Industrial Mining Co. must be named as an additional insured on General Liability and Automobile Liability.  A certificate of insurance from your insurance company evidencing that the proper coverage is in place must be on file.  It should also provide for a 60-day notice of cancellation, non-renewal or material change in coverage.
